Less than 1/2 pts in ā€œno IVCfā€ group were on AC. I agree IVCf recommended in acute VTE with contraindication to AC. So why were so many not on AC and did not get filter? This is where frailty and expected survival play a role in decisions but don’t get captured in retro reviews

Using death as a competing RF seems faulty when 30-50% of PE is only diagnosed post-mortem. Competing risk theory needs to assume completely different cause of death allowing pts and physicians a clearer indication of the risks that they face with each decision.

Filters have their place for those with acute VTE who cannot take anticoagulation and maybe have a survival benefit in those with massive PE and proximal DVT on anticoagulation. But their h/o complication rates means taking them out as soon as not needed.

Thanks to Ranya Sweis, MD keith for taking care of my 90 yo gma, performing an Impella assisted Shockwave lithotripsy DES of her distal LMCA. One wk post-discharge, she was able to go drop off her ballot. No excuses. Go vote!! Thanks NM Cardiovascular for making it possible.
