Intraoperative Consultation – Last Friday: 32-year-old woman with a retroperitoneal mass, suspected lymphoma based on CT findings. Touch imprint cytology, along with histological slide (similar to frozen section) and β-catenin IHC. #Pathology #Cytology #IntraoperativeConsultation

Three distinct faces of anaplastic thyroid carcinoma - all from the same specimen. These tumors can show remarkable heterogeneity, especially after neoadjuvant treatment. Beware the diagnostic traps!

Fun tip for budding #pathologists! When you see “shredded 🥕 ” (or zucchini or your favorite 🥗) in an esophageal biopsy - take a closer look! Candida loves to lurk around the desquamated debris, often on surface but can be detached too #GIpath Specials in the next tweet

Here in the 305, we know syphilis proctitis. On immunostaining, It's is easier to interpret AEC (3-Amino-9-Ethylcarbazole); it produces red staining of the organisms. The brown DAB (3,3' Diaminobenzidine) staining in organisms appears similar to normal anal melanocytic processes.

Bladder amyloidoma - Rare but striking! Look for: 🔹 Amorphous eosinophilic deposits 🔹 Subepithelial localization 🔹 Apple-green birefringence on Congo red under polarized light 🔹 Often mimics urothelial carcinoma clinically and radiologically #Uropathology #GUPath #Amyloidosis

Chest skin lesion, 25 yo young man. Langerin, CD1a, BRAF, ALK and OCT2 are negative in the histiocytes. S100 weakly positive in only part of them. What other stain would you do? Open comments for the answer

I find Poorly Differentiated Thyroid Carcinoma v. High Grade Differentiated Thyroid Carcinoma 😵‍💫 Former meets Turin Criteria Latter does not; but has higher mite count and WITH nuclear/architectural features of PTC or Follicular carcinoma. Prognosis of Latter can be worse

3 yo boy. Past medical history of B-ALL, KMT2A-R at age of one. S/P ASCT. Now presents with a paraspinal mass. Negative stains: CD19, CD79a, CD20, PAX5, TdT, CD3, CD34, CD117, MPO. FISH on fresh tissue shows 100% KMT2A-R. What do you think is happening here? Answer in comments 😉

36 yo female, inguinal lymph node. Look at the beautiful BCL2 negative microfillcles: H&E, CD20, CD3 and BCL2. What immunostain besides CD10 and BCL6 would you expect to be positive? Answer inside 😉
