A 26-year-old-female, obese with no other comorbids was hospitalised with fever and gradually progressive exertional dyspnea of 2 weeks duration - TUS revealed multiseptated effusion on right side- chest radiograph (ignore light artefact) is shown:

Post-procedure radiograph showed some remaining fluid collections- One injection streptokinase (1.5 mu) was divided in 3 equal doses and injected intrapleurally once daily that lead to removal of 1200 ml fluid volume-

Along with clinical improvement in dyspnea, there was much improvement in radiological opacity and drain was successfully removed-

Fluid was lymphocytic exudate and Histopathology was consistent with the diagnosis of pleural TB-
