Wow – 😮 Those 3 ECGs on the right panel show post-infarct Ventricular Tachycardia👇 But the QRS is too narrow…isn’t it🤔? It looks like SVT❗️ We Liverpool Heart Rhythm use detailed 3D/Ripple mapping Johnson & Johnson MedTech | UK & Ireland 🇬🇧 🇮🇪 to postulate the mechanism Heart Rhythm Case Reports #septalscar #HisPurkinje

Our new work: Uniform slow conduction during sinus rhythm and low voltage / low voltage gradient ΔV/V characterize the VT isthmus location is at preprint stage in HR sciencedirect.com/science/articl… And may be of interest for EP substrate mapping. #EPeeps #Cardiology #CardioTwitter #heart

⚡️Nice Fo8 “shared” isthmus supporting 2 VTs in this post MI endo-inferior scar #Optrell Liverpool Heart Rhythm. But we're missing 60ms TCL ⚡️LAT map interpolates activation, creating the impression of complete endo reentry ⚡️"Ripple silence" - warning sign you may need to map elsewhere

Dramatic RV paced ILAM. Multiple breakouts through diseased septum followed by activation of a narrow corridor within 2-3mm of WT on inHEART imaging. Case by Bishnu Dhakal MD FHRS of MUSC Electrophysiology. More info in comments.

Congratulations Juliana Pérez Pinzón, MD on this important work highlighting the incidence of pericarditis following PFA PVI. Predictors of post-ablation pericarditis were the same for RFA and PFA. Incidence was higher than in MANIFEST-PF and EU-PORIA but similar to real world cohorts.

In VT substrate mapping, wavefront direction is critical! Here is sinus, RV pacing, and LV pacing all at the same location on the inferior scar. The interaction of the wavefront direction with fixed lines of block create the split and late potentials. #ablateVT BIDMC VT Center
