Moving ward areas has not stopped our whole team caring for the patients. Everyone takes responsibility for making sure that our patients feel safe and supported #teamworkmakesthedreamwork

When patients have medications that may be new to staff, it is important that we share knowledge and teach each other #keepingpatientssafe #staffeducation

Through positive engagement and support from the teams, we have now signed off over a third of our staff as competent in completing Criteria Led Discharges. This has been achieved through real patient discharges and simulation to really embed the process #CLD #oneteam #success

Easter baking competition today on the ward. Two of our lovely visitors agreed to judge the contest. Well done Jason (our lovely chaplain) and his wife for being well deserved winners #bakeoff #cakeforall

Criteria led discharge badges are now being circulated on the unit to all Nurses that are CLD competent and the MDT when they independently identify appropriate patients. Little recognitions for the amazing work being done on the unit #Empowerment #teamwork #CLD

Excellent practice noted of HCA Maria whilst completing intentional rounding. Discussing and engaging with both the patient and her relative, allowed for a meaningful assessment to ensure she was comfortable and had no immediate needs
