"It becomes a self-perpetuating spiral. If people with frailty spend longer in hospital, stay in discharge-to-assess beds for longer and get insufficient rehab and therapy, they are going to have higher care needs at the end" hsj.co.uk/integrated-car… Health Service Journal British Geriatrics Society Dave West

I’ve been thinking on continued failure of the NHS to share records with patients. I think it’s important & will change the nature of healthcare interactions. These 2 things can be true: it’s too busy for such a big change & it’s important to do. If not now, then when?

Something I rarely see mentioned is that in many areas adult social care is the biggest single source of employment, bigger than the NHS, and the money spent on employing people in social care returns to those local economies.

Is this a good idea? Where is the care plan? Where is the care? What else isnt getting done? This is a list of tasks but no planned care (😬) or RNs managing care it there are few options. Its sad to see care as tasks, its even sadder to see how many people think this is OK.

If I ever touch someone's soul a one hundredth of how much #RyuichiSakamoto touched mine, then I will be completely accomplished.
