Further addition to plentiful evidence of the impact of perineural invasion on PDAC outcomes, even in early tumours. Nerves are critical in PDAC progression & PNI was previously described in minute 2mm tumours! The real question now is how do we exploit this to improve outcomes?

👇 💯. We are blessed with expert IR,endoscopy, ICU, anesthesia, dietitians, physio, nurses on wards seeing many of those complex cases everyday and knowing when something is not right,I don't see how our HPB surgery outcomes can be separated from this expertise!

No difference, but both are far from where we want to be! Seems to me that the issue is not MIS versus open, it's the pancreaticoududenectomy itself, much more to it than we already know perhaps? Optimised perioperative approach? Better rescue ?

Thank you 🙏 Definitely one of my pet peeves, people think giving a 40mmol of k in a bag of fluid will correct hypokalemia !! The way I remember it: 200-400mmol of k to raise serm level by 1.0 AND IV supp can't beat sandok if tolerated, unless in a big vein in ICU!
