"The big keratin eater" - Occasionally, the only clue to cystic met of a SQCC (especially in a neck node) would be the presence of multinucleated histiocytes with engulfed keratinized cell fragments. (The latter would not be seen in a branchial cleft cyst - the major DD).

Polypoid High-grade Dysplasia / Intracholecystic Papillary Neoplasm (ICPN) of the #Gallbladder with mixed cell lineage to include both gastric and pyloric as well as scattered goblet cells. The majority of these neoplasms harbor high-grade dysplasia. #GIPath #Pathology

Pancreatic neuroendocrine tumor (PanNET), WHO grade 1, 2.3 cm, ATRX mutant (& alternative lengthening of telomeres [ALT]). ALT-pos. NETs often have venous invasion (w/ tumor thrombi), leading to metastases #Pancreas #NET #PancreaticCancer #NeuroendocrineTumor #GIPath #Pathology

Intraductal Oncocytic Papillary Neoplasm (IOPN), #HGD, of the main & branch ducts in the #Pancreas. Interesting story, FNA in 2010 when 1.8 cm & repeat testing found a PRKACA fusion. The patient was asked to f/u & now 6.0 cm. #Pathology #GIPath #PancreaticCancer #PancreaticCyst

This unusual-appearing pancreatic ductal adenocarcinoma has a clear cell phenotype; we considered spread from a renal cell carcinoma and found loss of DPC4/SMAD4 helpful, as noted years ago. PMID: 10623651; PMCID: PMC1868651.

Unusual appearance of mucinous adenocarcinoma of lung with a large glandular structure almost completely surrounding an arteriole. I think that this glandular structure is likely within a lymphatic. Opinions? #pulmpath
