I think they interpret "fatigue" in terms of their own experiences of being tired from work. They feel tired but they can carry on working. Cognitive dysfunction is difficult to comprehend. Orthostatic intolerance is still just seen as feeling lightheaded when standing up.

Many doctors need a lot more education to even begin understanding how severe and limiting these problems are and to stop interpreting them from their own experience as a healthy person.
