LV summit PVC ablation...Are we safe if distance > 5 mm? Nagashima showed that coronary damage can happen even if distance > 5 mm (ahajournals.org/doi/10.1161/CI…). Here is our case and is this coronary spasm or damage? Settled after nitro and final image showed some coronary narrowing.

s/p PVI (2024) with very wide antral isolation. Note the corridor in the posterior wall towards the roof. Pt came with AT and ended up being roof-dependent AFL. Terminated while ablating the roof. dr_alsertdemir #Epeeps
