Lots of great replies. Rhythm here is NSVT, but what's the cause & the most important Tx? The answer lies in looking at the native beats: Patient had a large PE, needs anticoagulation! Detailed explanation is #FOAMed at ecgweekly.com/weekly-workout…

Full in-patient workup, and all she had was PE. It's not commonly taught, but PEs very often cause T-wave changes, can also cause STE or depression, narrow or wide tachys, rightward axis.

2️⃣ Case 2: Chest pain, not a classic STEMI? Look for Aslanger pattern ECG clues: • STE in lead III, not in II/aVF • STE in V1/V2 • STD in V4–V6 with terminally positive T 🧠 Think of Lead III and V1 as contiguous leads!
