Great way to tackle complex PSAs. Less morbidity than open repair and much better for the patient than putting a covered stent.

The love (patient comfort, safety 🙌) and hate (horrendous support with Ca/tortuosity) relationship with Radial for complex PCI continues. It mostly starts ok, until the guide is somewhere in the mid aorta 😡 and then you wish you had gone groin. Who disagrees and why?

Elderly male, Severe ischemic CM, native LAD occluded, recurrent admissions with NSTEMI/CHF. How would you manage?

Follow-up on this case. Functioning LIMA to LAD. Retro radial crossing of SCA CTO 0.018, reversed system. Radial to wire LIMA for protection (diseased ostium). 9mm BE stent. LIMA ostium treated at the end. This case was for angina/NSTEMI. Curious to hear VS colleagues’ approach.

Ostial RCAs are challenging, including “nailing” the ostium. Previously missed ostial RCA case now with ISR of the stent and severe actual ostial stenosis. A few tips: 1. Extreme LAO (>50) 2. Note CA on sinus 3. Double wiring as a marker 4. Mark with IVUS. 5. Flare at the end
