What’s in cryoprecipitate? FVIII, VWF, Fibrinogen and FXIII Each unit of Cryo increases Fibrinogen by 6-8 mg/dl Hence 10 unit will give a bump of 60-80 mg. If Fibrinogen <100, 10 unit will bump it up to 160-180 provided no ongoing consumption. In DIC recheck. In APL keep>150.

DIC vs. Primary Fibrinolysis. DIC is fibrinolysis after formation of Fibrin polymer. Both Fibrin Split Products and D-Dimers are elevated. Primary fibrinolysis is before polymerization. So FSP high but D Dimer normal. I saw a patient with snake bite and Primary Fibrinolysis

BK Virus is Bladder-Kidney tropic but the name derived from the initials of the first patient it was seen. Causes hemorrhagic cystitis post transplant. Besides bladder wash, three way Foley, cystoscopic cauterization: Cipro, Cidofovir, Leflunomide, BK CTL can be tried.

Purple top tubes are meant for drawing CBC. The bottom of the purple top tube is coated with a white residue. That’s EDTA-works as an anti coagulant by chelating out Calcium that’s needed for coagulation. Calcium is integral to coagulation. Calcium is the Clotting Factor 4.
