Overall this study adds to the research on CWF. Findings were: β€’ 3% fewer invasive treatments in F group β€’ Mean DMFT in the F group was 2% lower β€’ savings in NHS treatment costs in F group of Β£22.26 per person. β€’ The predicted ROI of 36% ROI between 2010 and 2020.

Limitations identified in the commentary were; data extracted were not validated against an epidemiological standard study looked at patients accessing NHS dental care, so it is representative only of those engaging with services. But other limitations within the study are:

The F cohort had a greater proportion of people from the most deprived quintile. Dentists have differing views on clinical intervention & this will impact on care provided. Privatisation of dental care means NHS data is not representative of disease experience in the population.

The study compares people receiving more than 0.7mg F/l with those receiving less than 0.7mg F/l. This may obscure benefits. The PHE Fluoridation Monitoring Report 2018 compared β‰₯0.7mg/l with <0.2mg/l. This comparison would have been more appropriate.

The F experience of the participants before 2010 is not considered. It is probable that some of the subjects in the fluoridated areas moved there from non-fluoridated areas before their first course of NHS dental care during the window 2010-20 & vice versa

Overall Lotus described CWF for adults as effective and cost effective but the study limitations may greatly underestimate the impact of fluoridation.
