This is for anyone writing a policy document on UK emergency care. 1. How does it help an older person on a trolley in a corridor at 2am on Sat morning? 2. How does it help a person in MH crisis needing admission on day 2? If it doesn’t, start again.

Fundamentally, this document encapsulates deep misunderstanding about Medicine. We TEACH medical knowledge in a sliced and diced up way, because the subject is vast and one has to arrange knowledge somehow. The PRACTICE of Medicine is essentially COGNITIVE. How we think 9/

Thank you Wes Streeting for responding so quickly to our concerns about ED waiting times/incentives. We look forward to working together to #ResuscitateEmergencyCare Adrian Boyle

#ICYMI: In the morning, RCEM expressed concerns about the unintended risks of incentivising ED waiting time targets, highlighting that it might leave some of the most vulnerable patients waiting the longest. That afternoon, Wes Streeting replied in the House of Commons. Watch

It almost sounds like Wes gets it. Silly incentives and targets are set when people who fundamentally don’t understand EM (and don’t have the curiosity or humility to speak to those that do) try to ‘fix’ EM. Here’s hoping this marks a change in focus. Time will tell.

Ken Milne MD @GatewayRuralH Louella Vaughan Kay M. Dingwell🍁🩺🏳️‍🌈 @SRPCanada CAEP Dr. Katharine Smart Michelle Cohen, MD alan drummond Dr. Brian Goldman André Picard Jonathan Sher The major cause of ER overcrowding is the lack of fully funded & staffed in-patient beds. It always has been. EDs aren't designed to hold patients long-term. It's also less safe for the admitted pt. Walk-ins are easy. It is time to actually spend money on admitted pt areas.

Why the NHS is skint in one picture. £8.62 for the ONLY purchasable option for crayons for a children’s waiting room. 🤦🏻‍♀️. Fortunately staff spend their time cutting them in half to make them go a little further.

Consultants SHOULD see pts on their own! Using resident doctors just to drive computers is awful! Work fit for only someone else! The hoo-ha over derogations has prompted me think a bit more about what ACTUALLY happens on a WR. Thread about cognition + safety 🧵 1/
