In a way, NGAL and MBGC by urine sediment are injury markers and very low FENa is a functional marker. At least 4-5 new patients the last few weeks have had the same story… -Hypertensive on 3 agents -semaglutide gets added -loses 30 lbs - blood pressure too tightly controlled -cr goes up - pcp places referral. Good problem, I guess but patients are confused and anxious
