What immediate lessons for mental health from the Nottingham inquiry? 1) We are too reluctant to intervene when signs of risk are becoming clear, citing “least restriction”. 2) We do not accept what families tell us, our fleeting observations counting for more.

3) We are too quick to sign up to the latest therapeutic fashion - “positive risk-taking” in this case but not too long ago, the SIM fiasco.

4) We confuse clinical recovery with insight. The latter is key to long-term risk. Insight is linked to acceptance of need for treatment. 5) We are reluctant to diagnose schizophrenia, for understandable anti-stigma reasons, even when it is obvious.

6) Involvement of private sector & out of area admissions may add to risk. 7) We - professionals - still regard the use of legal powers as hostile to a patient when the treatment they bring may be only way a patient can live what the rest of us expect as a normal life.

Staff are doing what they can in the face of rising demand. Blaming individuals for decisions made in a system overwhelmed by demand is misplaced.

Public statements from authorities will say this must never happen again. But it will. The risks are preventable but ingrained. Realistic pubic messaging is part of the solution.
