Lots of chatter about potential outcome of The Leng review Wherever it lands? The problem is now..finances If you had 50K to spend on staff- unless ring fenced (which can be legally challenged by any other group)- there is no feasible reason to hire a PA/AA above -forget doctors-

£2.2bn has been allocated in support funding as ICSs’ final financial plans say they will not OVERSPEND more than this🤯 Previous years have seen large deficits just a few months into the year GPs have seen a £2.3bn cut in annual funding, no support or just hand back contracts

Mark Steggles Will they be funded though ? And at the right level. Its impossible to equate a % uplift on one cost line to a £ input on one income line. As @DrKatie.BSky.Social says this time it needs to be across all areas

One of the questions that some of my GP colleagues ask is why they can’t see the x-rays they request for their patients. They will receive a written report from a radiologist but generally can’t view the image. With modern NHS IT systems, this should be possible.

15yrs ago waiting lists were minimal in the NHS If I referred you for an urgent appt you were seen within 2-3 weeks, routine in 2-3months There had been investment in health care over the previous 10yrs Employment was high, national debt had been falling It’s possible again

I’d like : A computer that doesn’t take 10mins to switch on An operating system that doesn’t crash 3-4 times a day A system that alerts me when tests/referrals I’ve requested haven’t been done Tony Blair: Britain should have AI doctors and nurses thetimes.co.uk/article/tony-b…

When people say “Outpatient care in hospitals must be transformed” They basically mean “More shifting of care to primary care but without the resources” If you can’t be honest with your workforce? Don’t expect productivity Thanks for coming to my Wednesday TED talk

My take from the literature: 1/5 The fragmentation of job roles into tasks can increase coordination costs, put a higher burden on senior staff who have to oversee the work of juniors and risk disrupting continuity of care.

Alison Moore @alijanemoore also on Bluesky Nigel Edwards also in general practice (or district nursing or practice nursing or other roles where you develop a long term therapeutic relationship and trust over years) some of the "simple" "tasks" are a chance to foster/reinforce that continuity and to "check in"
