54-yr-old female w/ MVA 4 yrs ago & persistent, posterolateral upper neck pain. x-rays read as normal. AI movement analysis shows Posterior Vertical Instability Index is abnormal w/ posterior disc widening in flexion. This would be almost impossible to detect w/o the AI analysis

The answer should’ve been sacroplasty. 67yo s/p pelvic XRT, developed sacral, insufficiency fractures, tx w/ opioids, then more opioids, then L4 to ilium fusion, then pain pump + oral opioids. 5yrs later still has nonunion fxs. Should’ve had sacroplasty. Still can but w/ screws

It’s bad enough that delayed flights on United Airlines caused me to miss the Taiwan Neurosurgical Society meeting, but United failed to: 1. Give a reason for the flight delay (1st flight of the day inbound from Albuquerque) 2. Find other flights prior to the end of the meeting.

3. Notify us of the flight delay until after boarding time. 4. Allow a conversation with customer service in this regard until tomorrow. 5. Send me an email explaining the situation other than the flight cancellation confirmation.

Follow up to this case, Sacroplasty was performed with 10 cc of cement on each side plus screw spanning the gapped left sided sacral alar fracture. Patient is doing much better and I will see him at two weeks then at eight weeks follow up. osteoporosis treatment is underway

No mention of an obvious vertebral compression fracture in this report. Severe height loss with signal changes are described as nonspecific, & the signal changes are described as “suggested”. Because of this nonsensical report the fracture was not known to exist #comeonman

CMS makes some of their target list based on hit pieces that are not grounded in science, the literature, or reality. check out this article w/ targets on minimally invasive lumbar decompression & vertebral augmentation, both of which have great literature support and outcomes.

Instead of “nonspecific” marrow changes at L4 and S2 please name the diagnoses which are a fractured L4 inferior endplate and hemangioma at S2. Describing it this way makes getting the appropriate treatment (Kyphoplasty at L4) approved difficult.
