Care produced together with patients in region jonkoping starts with the patient identifying what they can do for themselves with their resources and networks and then seeing how the healthcare system can support them. Love this strengths-based approach #Quality2022

The "Best place to live" approach in jonkoping aligns all social determinants of health including education, leisure, environment and beyond. This hopeful, inspiring approach seems absent in England - perhaps we need not just joy in work but hope? #Quality2022 James Mountford šŸ‡ŗšŸ‡¦

"Integrated care is not a new organisation, it's a way of thinking with primary health care at the centre" Empty circle represents the personalised aspect for what that patient needs - for me it would be yoga šŸ§˜ā€ā™€ļø #Quality2022

Much of this work can happen at the frontline but demonstrating the "seriousness" of this work using an academic approach to develop a framework engages politicians. Then combined with PDSA approach to testing and tweaking. Love to see HSR and QI combined! #Quality2022

"The accountability for providing high quality care lies with the profession but the necessary conditions are set politically" So true. Really interesting ideas for the political level to consider to facilitate healthcare improvement #Quality2022
