#PennsyInterventions CaseFiles: How would you approach this complex retrieval of angulated 10 yr old IVC filter with externalized embedded apex/hook and penetrating struts? Phila Angio Club Penn Cardiology Fellows Penn Cardiology Academy for CardioVascular Collaboration ReVascMed

Here’s how we accomplished this percutaneously step-by-step: 1/ first we utilized #ChainLink technique with combo loop snare from above and below to straighten the filter @VVargheseDO

2/ then we brought #CavaClear from Philips Healthcare to ablate embedded fibrotic tissue from hook and tip of filter to allow capture into the large 16 Fr sheath from Cook Medical

3/ next, with adequate laser ablation, the filter was captured and retrieved successfully

4/ finally externalized the retrieved filter with demonstration of extensive residual tissue around the apex of filter

5/ final venogram with no residual contrast extravasation
