Infarct necrosis important to separate from true tumour necrosis in NETs.2 show true necrosis(abrupt transition, karyorrhectic debris, comedo-like)& 2 show infarct(gradual transition, zonal granulation tissue/myobroblasts) If you think easy -> retweet If you think hard -> like

Hello #BreastPath friends! Spotted an interesting staining pattern—CK5/6 (3rd pic) is trying to tell me something in this intraductal proliferation, but I don’t speak its language. ER (last pic) is also in the mix. Awesome brains?

This insane epithelioid hemangioendothelioma arose in the neck and has a lymphoid cuff - both of which mimic epithelioid hemangioma. CAMTA is confirmatory. Note the intracytoplasmic lumen with an erythrocyte (arrow).

Enteric yolk sac tumor This extensively necrotic anterior abdominal mass shows positive CDX2, KRT8/18, and SALL4, all features identical to enteroblastic Ca. If enteric type yolk sac tumor presents as a metastasis of unknown origin, only isochromosome 12p testing tells the truth.

THYROID with metastatic lung "Small Cell Carcinoma". Macrophages provide a good size reference. DD - Medullary CA uncommonly may have a "small cell" phenotype as well as mets from other neuroendocrine tumors (I've seen at least two cases of Merkel cell CA mets to thyroid). (FNA)
