Measure tendon length. Drill longer tibial tunnel (guide has length measurement) recess in femoral tunnel as needed. No worries! Two incision works as well!

Don’t worry about the angle. Look at the tunnel length on the drill guide. Can always get the correct length tunnels. Angle too dependent on portal position and what angle you have your hand at.

Really depends. What is there anatomy? What is there sport? Where are we in the season? Youth, high school, college, pro? Most can do fine but sometimes need to pull the trigger. Mpfl, tto, combined. Rehab 4-6 months but can take longer. Always a discussion.
