Impressive viral inclusions in this case of BK nephropathy. Some made it all the way to Bowman's capsule. Also prominent granular TBM deposits and nuclear staining with IgG. #renalpath #nephropath

Allograft biopsy for rising serum Cr. At first glance, seemed like vessels had severe hyalinosis c/w chronic CNI tox. But closer looks showed associated giant cells and waxy appearance. Congo red confirmed amyloid. Amyloid A IHC positive. Mass spec pending. #renalpath #kidneypath

Never seen this: ? Whipple’s disease in the kidney. Patchy accumulation of aggregates of PAS-positive rods in Bowman’s space, few capillary loops, interstitium & tubules. Initially thought of crystal storing histiocytosis but IF negative for Ig. EM showed bacteria in tubules.
