Matt Siuba Very true Matt! Depends what you mean by maintenance fluid though. People must get their daily water requirement preferably orally, then NG and only IV if this not possible. And not >1 mmol Na/kg/day. All infusion intake should be taken into account. #turningthetide

The poor faculty/resident salaries in state medical colleges are a scandal and drives faculty into the arms of private practice. Answer - raise pay (tho' unlikely to match pvt sector) Using unchecked promotions as a retention strategy is a bad policy and bad in practice. ++

Research and education should receive priority and protected time. Young faculty members should have career development opportunities - in research skills and models of education. Protected time for research & supplementing salary with grants should be considered. ++

So as a junior myeloma faculty, I see a lot of consults for MGUS. Heres a tweetorial on how to interpret light chain values in the setting of kidney disease, as light chains are often checked to look for plasma cell disorders when patients have CKD. 🧵 #mmsm #MedTwitter

Hypocomplementemia can occur in many types of GNs, however, you can have severe glomerular disease in the presence of normal serum complement levels too. #KidneyWk

This has great public health implications, we need more epidemiological surveys to better understand the spread and trend of this condition to Try and identify cause(s) Hint - there may be more than one And develop appropriate interventions - at community and individual levels

We must address the diagnostic and therapeutic nihilism that puts the affected individuals at double disadvantage First - they they get the disease and Second - they are neglected by the system, do not receive proper work up or treatment to slow disease progression
