Bringing #healthcare closer to home for community members is at the heart of the work being done by Weeneebayko Area Health Authority and University Health Network. 🧵⬇️

With a portable echocardiogram in tow, our team cared for 100+ community members and trialed a new digital sleep apnea screening device. Regular in-person visits, combined with Medly, virtual care, and remote management, support earlier detection and prevention of #heartfailure.
