Employed physicians frustrate me at times. They complain about autonomy and burnout… yet stay silent on issues like Medicare reimbursement. They don’t seem to see the irony. HJYouga made a terrific point recently: Employed physicians are increasingly disconnected from how

Except physicians. Despite all these things being true (experience, established, technical skill, etc), you are bound by the same reimbursement, referral, and administrative monopolies (hospitals, insurance, regulation) that you were on day 1 after graduation.

And how would you feel, Max Jordan Nguemeni , if a teacher or firefighter told you to 'be quiet and enjoy your enormous salary?' Trust me, physicians (probably including you) don't get paid a penny more than they earn (usually much less).

Remember, primary care is reimbursed really well (facility fees to hospitals are enormous) -- it's just the professional fees to the physicians that are abysmal. Conveniently, this difference allows hospitals (while profiting massively) to delude MDs into believing they lose $.

Primary care physicians have two options. The first is to advocate for an increase in reimbursement, and this will never happen. The second is to give themselves a raise and become a direct primary care (DPC) physician. That way, you set your fees and determine how many

'Teach' is the operative word. They 'teach' med students how to run medical practices, too. Just creating another dumb box to check, more millions to admin/committees, and more time away from patients.

Sanjay S. Dhall, M.D. Scott Becker Thank you immensely for saying this. As an ER doc, if I publicly drop hints that I think admin is the problem, I'm immediately at risk of losing my job. I'm an expendable cog. Whereas when the specialties that administrators need to generate their facility fees and margins

Great cohort. Although PCPs could use your help a lot more than spine surgeons. Whoever has been at that table....they need to go.
