SDH deficient RCCs appear to have originated from macula densa cells. These cells play key sensory and regulatory functions, similar to the function of cells that give rise to the neural crest derived SDH deficient pheochromocytomas and paragangliomas.

One marker of macula densa cells, PAPPA2, appears retained in all SDH deficient RCCs. As diagnosis through morphology and immunohistochemical staining is not always conclusive, positive PAPPA2 staining may prove a valuable adjunct. #gupath

We saw a striking lack of immune infiltration in all of these tumours, possibly via the combined effects of PAPPA2, TGF-b and PD-L1 expression. In this context, traditional immune checkpoint inhibitors alone may fail, requiring tailored combination therapy.
