PSA to all PAD interventionalists. Please, please, PLEASE send for a surgical evaluation ANY patient with a long segment SFA-Pop occlusion and a proximal tibial that is open. A full metal jacket from the CFA INTO the last tibial either leads to amp or a high risk bypass.

How to parachute. It’s really an important technique to learn. The most important things to have. Long suture. 48 inch 4-0 BB for proximal anastomosis. I put 4-5 stitches in, then bring down graft. Especially useful in deep holes. 1st year fellow doing end to end to beveled aorta

Adam W. Beck Great question Adam. I’ve used all. Younger I used PTFE ringed but I felt it created some mild dysphagia. Used getinge fusion once or twice, no real issues, except sewing it is a bit stiff. I’ve gone to Dacron (Terumo) for this last 2 years, my preferred. And you?

When you sew in surgery just use good technique. Whether the thumb is in or out of the needle driver has little bearing on your skill set or outcomes. Some palm and some put fingers in the holes. Both work fine.
