Kidneys adapt in response to damage in its attempt to preserve normal function. Healthy nephrons take over the work of sick nephrons (just like you covering for your sick officemate). That’s why in early CKD, kidneys look “fine”. 1/2

Uric acid crystals/microtophi: - associated with gout - occur in deep medulla - collecting ducts - can migrate from tubular lumens to interstitium - can have a granulomatous reaction - basophilic and polarizing when examining frozen sections From Dr. Biederman @renalpathsoc

Thx Dan! Yes! So important to distinguish for appropriate patient care! ‘Shoulder’ of PUC is NCCN intermediate risk vs high risk for UCIS. It matters! 🔬

4: STAGE- tumor encroaches SV/EJ epith, worrisome for pT3b but not Dx-ic especially on prostate bx particularly not flagged as coming from SV ✅for these, I add comment & suggest they correlate with rads (FYI final RP here was pT3b) ✅can use PAX8 to highlight EV/EJ (👇🏽example)
