I find that glaucoma is easiest to manage if you anthropomorphize the disease and imagine you are fighting an extraordinarily diabolical villain - and try to stay a few steps ahead.

Postop from my colleague, on a rho kinase inhibitor. My belief is that rho kinase inhibits the interaction of actin and myosin, which do exist within the corneal epithelial cells, creating this pattern instead of the typical microcysts. Are there other explanations?

Had a nice experience in the operating room with the OCULUS, Inc. BIOM ready hands free surgical gonioscopic prism. It was easier to use than I had expected, and having both hands free was nice.

I have long held that PGAs don’t cause CME & that many glaucoma patients lose vision while being denied a PGA despite PGAs A+efficacy. Turns out PGAs have the lowest risk of causing CME. Thank you Dr. Zhou et al for this A+ paper. Let’s let dogma go. pubmed.ncbi.nlm.nih.gov/39122155/#:~:t…).
