Patient taking quinine has laboratory TMA findings and negative serologic studies. Biopsy shows severe tubular injury and glomerular-limited TMA with extensive thrombosis and focal endocapillary pmns due to quinine-dependent antibodies reactive with platelets and neutrophils.

Middle age man, hgbA1c 5.8, glucose 111, hematocrit 9.2, UPC 9.6g/g, Screat 5. Biopsy shows advanced nodular glomerulosclerosis, then found to have diabetic retinopathy. Glycated albumin and glucose tolerance test can further assess for diabetes with anemia and normal A1c.
