Experience and expertise is there a difference? How does one develop expertise in ones chosen field of clinical care? Twitter is famous for it’s echo chambers and camps of belief and support. There seems to be a trend that leans away from experience yet not expertise? A thread

A brief thread on recovery of motor deficits after Dc herniation , the evidence for early surgery is not clear, evidence for wait and see seems comparable in recovery type and rates pubmed.ncbi.nlm.nih.gov/12358369/ Recovery of motor deficits after microdiscectomy for lumbar disc hernia

I have been pondering on the Sacroiliac Joint and some clinicians fascination with it. I think that it is the source of more “evidence based ostriches” than any other joint. Yes a brief thread The SI joint and pelvis are not a “pubics cube” we do need to stop the madness.

Interesting study that may challenge some disc centric views on the effect of prolonged sitting. It took just 20 seconds of movement every 15 minutes over four hours of sitting to reverse any effects of the sitting stresses. So around 1.5 minutes of motion over four hours

A ponderance brief thread: 🧵 If you are going to do a neurologic examination why? I am going to suggest that the reason to do a neurologic exam is driven by the patients history. The INTENT of a neuro exam is to set a baseline.Baselines can be used to monitor any deficits

As an advocate for choosing our clinical messaging wisely, but “word shielding “ can go too far. We talk about exposing people to load that hurt, shouldn’t we do the same with words in context? CONTEXT is important. When discussing MRI reports the CONTEXT of the words matters 🧵

Pain is weird….🤨 Just saw a patient who had severe sciatica from a disc extrusion for 1 year… …then was rear ended in a car and within 48 hours his pain was gone…..a year later he is still pain free I say again: Pain is weird 🤷‍♂️

New systematic review confirms AGAIN the findings of Brinjikji et al. 2015. Multiple findings on MRI are CORRELATED with pain. I wrote about it a while ago (link in comments).
