The subventricular zone harbors multipotent neural stem cells that can mobilize in response to damage. Advanced MRI can explore this in vivo. Our study indicates early microstructural changes in the subventricular zone in pediatric multiple sclerosis, linked to brain damage.

A paradigm shift is needed in MS RCTs. The evolving understanding of MS highlights the limitations of traditional RCT endpoints in capturing the complexity of the disease. Pathology-driven endpoints should become increasingly relevant to test the efficacy of emerging therapies.
