📜 New paper giorgia previdoli @bethfylan @MedicinesDavid Daniel Okeowo Jon Silcock 👉doi.org/10.1111/hex.13… How do older people with frailty experience managing multiple medicines at home? We interviewed 32 older people living with frailty. Our analysis produced five main themes:

Managing many medicines is a skilled job I didn't apply for: It is a skilled and safety-critical job demanding time and dedication. People manage medicines and dose changes, different formulations, timings, storage, and check medicines and monitor how they feel.

Medicines keep me going, but what happened to my life? People tried to balance the demands of their medicines regimens with the desire to continue their normal lives. Some adapted their regimens, e.g avoiding taking medicines at specific times, or stopped taking them altogether.

Managing medicines in an unclear system: People tried to mange medicines in a fragmented care system, which was difficult to access and navigate. Some felt overlooked, unheard, or misunderstood.

Support with medicines that makes my work easier: People described the practical and emotional support received from their own networks and from healthcare professionals. They valued good relationships and being recognised as an individual.

My medicines are familiar to me—there is nothing else I need(or want) to know: People described being familiar with medicines, taking them automatically (sometimes without knowing what they were), trusting healthcare teams without question & the systems that connected those teams

Our work highlights how older people with frailty at heightened risk of the impact of medicines errors are expected to perform complex work to safely self-manage multiple medicines at home. Such a safety-critical workload needs to be supported & people adequately prepared.
