Without the intranuclear inclusions, these "oncocytoid" cells would have been a bit difficult to be called "Papillary Thyroid Carcinoma" on FNA. Follow-up, "PTC, Tall Cell Subtype".

Adenocarcinoma, presenting a sharp contrast with the juxtaposed benign squamous cells in a lower esophagus brushing. Prominent solitary macronucleoli, lack of adhered PMNs and a clean slide background with absence of inflammatory debris rules out reactive condition. (ThinPrep)

Fairly straightforward to call this Pancreatic FNA "Malignant", but often harder to subclassify as a "Primary Anaplastic (undifferentiated) Carcinoma", without IHCs. (EUS-FNA, Pancreas)

"Synovial Sarcoma" is known for its bland monotonous cytomorphology on FNA. Brisk mitotic activity (as seen here) is always helpful. Confirmation with IHCs (and often FISH) is quite commonly practiced. (Soft tissue mass, Abdominal wall, FNA)

"Post-BCG granulomas" in voided urinary specimens may appear "atypical" to inexperienced eyes. Patient had a history of non-muscle invasive high-grade urothelial carcinoma. (SurePath)

It's quite rare to see such DENSE keratinization in a benign/metaplastic squamous cell. Often referred to as "Halloween Orange", this cytoplasmic feature is often observed in keratinizing Squamous Cell Carcinoma (Pap stain of an exfoliative lung specimen).
