Has "billing friendly" terminology replaced relevant clinical information in clinical documentation? "Acute metabolic encephalopathy" is meaningless to me - is pt disoriented? lethargic? obtunded? agitated? "Acute hypoxic respiratory failure" - can be on 2L NC or intubated?
