Let's be clear, though. "Knock[ing] out the middleman" here has virtually nothing to do with getting drug costs down. There's a reason pharmaceutical companies were lobbying so hard for PBM restrictions.

In the 2026 MA Advance Notice, CMS said that it may begin calibrating the MA risk adjustment model using encounter data (snippet below). Unfortunately, this change would not achieve what CMS hopes it would—and might well be a step in the wrong direction.

We already know where a lot of the waste, fraud, abuse is. Between Medicare Advantage, policing Medicare fraud, & enhanced tax collection efforts, there's big $$ to be saved. And it can even be done through legal means, a good amount with current authority.
