If you don't want a fistula from SpaceOAR placement, go see Daniel E Spratt There were at least three SpaceOAR-associated fistulas in Oct 2025 alone(!) accessdata.fda.gov/scripts/cdrh/c… Many reports of pain, abscess, radiation delays, and SpaceOAR poop-outage

Twice a day RT to 60 Gy is *the* standard of care for limited stage SCLC It should replace *the previous* #radonc standard of care: twice a day to 45 Gy If you don't (oftenly, persuadingly) offer twice a day RT for small cell #lungcancer ... rethink those life choices

The whole medical publishing ecosystem (free content, free peer review, but $$ to publish/read) is a bizarre, exploitative business model. Even if one believes clinicians owe some degree of unpaid "academic service," it’s absurd publishers profit so massively off our free labor.

“it’s the broken insurance industry, where NURSES spend nearly half their time fighting with insurance bureaucrats who say no to life-saving treatments” Haha. Nurses. Nurse peer to peers I reckon. I could have had my nurses doing the fighting the whole time?! Whewie.

As a practicing #radonc, I am aware that federal law proscribes that any time I get a treatment denied for a Medicare Advantage patient I can appeal. And if denied the denial will be reviewed by an independent reviewer (“Maximus”) who can reverse the insurance company’s decision

Theory: There would be about two or less proton treatment centers in US (at current proton machine prices) if protons reimbursed <100% IMRT rates

We've injected ~6 olympic size swimming pools of contrast material into Medicare beneficiaries over the last ~15y (about a megagray of CTs, too) auntminnie.com/practice-manag… The contrast agent material is becoming detectable in the water supply(!) link.springer.com/article/10.118…

Never meet your heroes, or obsessively read their essays or books, or be inspired to consider neurology as a specialty as a medical student, or hold in heart ‘Awakenings’ as a favorite movie
