. Tim Cook said it’s 10 yrs since #NAP4. I re-read the Foreword. I think anaesthetists have met the challenge I posed, but events have shown that systems thinking & simulation aimed at nontechnical behaviours are even more important. But how’s the rest of medicine doing?

Great example of how systems can make it easy to make a drug error, add in fatigue, overload, stress, interruptions or poor lighting on night duty to create a perfect storm. #Humanfactors

We tried this approach recently for some of our interview questions. It was great for candidates who like to think, research, consider and reflect before answering. These skills, which are just as valuable as rapid thinking, can otherwise be missed. It's better for inclusion too.

An eye opening experience from the iTRUST debriefing course at Simulation Services @ UHBW >Learned so much on structuring debriefs >Constructing questions to understand the learner's 'mental model'. >There's no such thing as nonjudgmental debriefs Wendy Collins Professional Education Practitioner Pauline.Rea @GlosProfEd

Such a wonderful and deeply moving article exploring the art of communicating well with families about the realities of resuscitation for critically unwell patients. Brilliant piece, Sunita, thank you
